Forms on this web site may require users to provide contact information. Contact information is only used when a staff member needs to reach the user regarding a submitted request.

When a page is requested from our web site, our web servers automatically recognize the user’s domain and IP address (an identifier for a computer or device that is used to route Internet traffic, or basically your “address” on the Internet.) This information may be used to obtain usage statistics or web site hits. We do not, however, collect or evaluate this information for a specific user.
