Portsmouth Public Library offers basic tax forms provided to us by the IRS and the State of Ohio.

Provided Forms:

  • Federal Instructions for 1040,1040A, & 1040EZ
  • Federal Forms 1040, 1040A, 1040EZ
  • Federal Form 1040 Schedules A, B, & EIC
  • Ohio Booklets
  • Ohio Forms IT 1040, IT 1040 EZ, SD 100

Provided forms are free of charge at all locations.

  • PPL staff may be able to print forms for $0.10 per page as long as patron knows the title or the form number.

Portsmouth Public Library CAN NOT help with tax preparation.

Tax Forms Online

  • Patrons can also go to the IRS website at www.irs.gov or to the Ohio Department of Taxation website at www.tax.ohio.gov to find & print forms.
